The Eastfield House scheme will provide 45 beds, as well as parking spaces, landscaped gardens, a café/bistro and activity lounges.
It will also include nature walkways, a sensory garden, and a pavilion.
Proposals have been submitted to South Oxfordshire District Council for the development — if the scheme is approved, construction is projected to start later this year.
Once completed, the care home will be let to Bracebridge Care Group for a minimum term of 25 years.
